2026 GSSSL and Blue Ridge Required Forms
The Greater Seattle Summer Swim League (GSSSL) requires all swimmer families to complete a set of participation forms before taking part in any team activities. These are league-mandated requirements — not specific to Blue Ridge — and apply to every club in the league.
In addition, Blue Ridge requires a photo and video release for all participants.
The following forms are included in this section:
- Code of Conduct — Acknowledgment of league behavioral standards, sportsmanship expectations, and eligibility rules
- Participation Form — Consent to participate, assumption of risk, and liability release
- Concussion Awareness — Acknowledgment of concussion risks, symptoms, and Washington State return-to-play requirements
- Photo and Video Release — Authorization for Blue Ridge to use images and recordings of your minor child in team-related communications and media
We are also collecting standard medical and emergency contact information for each swimmer as part of the registration process. This information is used solely by coaching staff and meet officials in the event of an emergency.
Please read each form carefully and sign where indicated. All four forms must be completed before your swimmer may participate in practices or meets.
